Daycare: Please download all forms below. Complete and mail to the above address with a check or money order in the amount of $35 payable to East Paws Doggie Daycare.

Grooming: clients who are NOT interested in Daycare please download all forms, complete and return.  You do NOT have to enclose registration fee.

Required for all Dogs:  

• All dogs MUST be treated with flea and tick protection (FRONTLINE PLUS).  We are NOT responsible for any dogs that leave our facility with fleas or ticks.  Any dogs that enters our facility who has fleas will be sent home immediately! 
• Veterinarian certificate of vaccination records. (We can not allow ANY dogs who are not immunized onto our premises)
